Technology Census Sweepstakes Official Rules
How to Enter: No purchase necessary. Entering or participating constitutes entrant's full and unconditional agreement to and acceptance of these Official Rules. The Dental Technology Census Sweepstakes begins April 1st, 2018, at 12:01AM Eastern Standard Time ("EST"), and ends December 31, 2018, at 11:59 PM EST. You must be a citizen of the United States of America and at least the age of majority in your jurisdiction of residence at the time of entry to be eligible for this Sweepstakes. You may enter the Sweepstakes by visiting www.technologycensus.com and completing the Dental Technology Census 2018. After fully completing the census, you will be automatically entered into the Sweepstakes. Limit of one entry per person.
The Sponsor and its parents, affiliates, divisions, and agencies, and each of the foregoing entities' respective employees, officers, directors, shareholders, and agents (the "Released Parties") are not responsible for incorrect or inaccurate entry information, human or electronic error, unauthorized human intervention, technical malfunction, lost/delayed data transmissions, omission, interruption, deletion, defect, line failures of any telephone network, computer equipment, software or any combination thereof, inability to access web sites, personal injury, or property damage due to participation in this Sweepstakes, or for late, lost, incomplete, stolen, misdirected, mechanically reproduced, illegible entries or postage due entries and/or mail, or any other problem or error that may occur, including in connection with any prize.
Eligibility: To be eligible to win, you must be: (i) a licensed dentist, (ii) a dental assistant, or (iii) an office manager that has been employed at a licensed dental office during the entire six (6) months immediately prior to entry. Employees, officers, and directors of Sponsor, its parent companies, affiliates, subsidiaries, third-party partners, advertising, promotion, and fulfillment agencies, their immediate family members, and persons living in their same household are not eligible to participate in the Sweepstakes. Void where prohibited. Federal, state, and local laws and regulations apply. Prizes will be awarded to the entrant's dental office, as identified by the entrant in his or her entry (the "Winner").
Random Drawings: Three Winners will be selected in a random drawing from all eligible entries received by Sponsor during the entry period (one Grand Prize Winner, one 2nd Prize Winner, and one 3rd Prize Winner). Sponsor's decisions on all matters pertaining to the Giveaway are final. Odds of winning depend on the total number of eligible entries received. The Winners will be notified by email or phone. Each prize Winner may be required to sign an affidavit of eligibility (which affirms that they have complied with these Official Rules), as well as a liability release and, where legal, a publicity release which, if issued, must be completed, signed, and returned within fourteen (14) days from date of issuance or the prize may be forfeited and awarded to an alternate Winner. Acceptance of a prize constitutes permission for the Sponsor to use Winner's name and likeness for advertising and promotional purposes without additional compensation unless prohibited by law. If, in the Sponsor's opinion (in its sole discretion), there is any suspicion or evidence of tampering or technological corruption of the Giveaway, or if the security, integrity, or proper play of the Sweepstakes is compromised in any way, the Sponsor reserves the right to void any suspect entries and/or modify, suspend, and/or terminate the Sweepstakes and award the prize using all non-suspect eligible entries received as of such termination date. CAUTION: ANY ATTEMPT BY AN ENTRANT OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF SPONSOR IS A VIOLATION OF CRIMINAL AND CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH ENTRANT TO THE FULLEST EXTENT PERMITTED BY LAW.

No prize substitution or transfer of prize permitted, except by Sponsor due to prize unavailability, in which case prize substitution will be of equal or greater value. An unclaimed prize will not be awarded. After being contacted via email or phone notification, a Winner will have fourteen (14) days to claim his or her prize. After the fourteen (14) days from time the email or phone notification is sent the Winner forfeits any right to claiming the prize. The prizes are being provided "as is". Sponsor will not replace any lost or stolen prize items. By accepting Grand Prize, Grand Prize Winner agrees not to resell or transfer the any portion of the Grand Prize to any third party for a period of 18 months from the date of award.
Disputes: Each entrant agrees that: (i) any and all disputes, claims, and causes of action arising out of or connected with the Sweepstakes, or any prize awarded, other than the administration of the Sweepstakes or the determination of Winner, shall be resolved individually, without resort to any form of class action, exclusively in the State or Federal Courts located in New Jersey; (ii) any and all claims, judgments, and awards shall be limited to actual out-of-pocket expenses incurred, including costs associated with entering the Sweepstakes but, in no event attorneys' fees; (iii) notwithstanding subsection (ii) above, it will release and hold harmless the Released Parties from and against any and all claims, actions and/or liability for any injuries, loss or damage of any kind arising from or in connection with participation in and/or entry into this Sweepstakes or use of any prize; and (iv) under no circumstances will entrant be permitted to obtain awards for and entrant hereby waives all rights to claim punitive, incidental, and consequential damages, and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased. SOME JURISDICTIONS DO NOT ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILTY FOR INCIDENTIAL OR CONSEQUENTIAL DAMAGES, SO THE ABOVE MAY NOT APPLY TO YOU. All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules or the rights and obligations of the entrant and Sponsor in connection with the Sweepstakes, shall be governed by the laws of the State of New Jersey without regard to the conflicts of law provisions therein.
General Conditions: Sweepstakes entrants agree to abide by the terms of these Official Rules and by the decisions of the Sponsor, which are final on all matters pertaining to the Sweepstakes. Sweepstakes is void where prohibited or restricted by laws, including but not limited to jurisdictions with laws that would require registration, disclosure, translation, filing, trust account, posting of a bond, or any other requirements that are not satisfied by these terms and conditions.
Winner List: The name of the Winners may be obtained after December 14th, 2018, by sending a self-addressed stamped envelope to: Inquantive Research Sweepstakes Winners List Request, P.O. Box 606, Marmora, NJ 08223. Vermont residents may omit stamp.
